It may be seven months since Michael Douglas beat throat cancer, but it seems the star is still struggling with his nicotine addiction.
According to the Mail Online, the Wall Street actor was seen smoking a hand-rolled cigarette while on board a yacht on a recent trip to the Italian Riviera.
His wife Catherine Zeta-Jones, who supported her husband through his cancer battle last year, was also pictured cigarette-in-hand during the couple's break.
Douglas announced that he was fighting advance stage throat cancer in August 2010, attributing it at the time to years of heavy smoking and previous alcohol abuse.
He underwent a gruelling chemotherapy and radiation treatment programme and lost 32lbs in the process.
Michael then announced the January that he had beaten the disease, telling NBC Today host Matt Lauer:
"I feel good. I feel relieved.
"The tumour is gone [and] the odds are with the tumour gone and what I know about this particular type of cancer, that I've got it beat."
A spokesman for Michael Douglas is yet to respond to the images posted by the website.
